SSIMED 3nd Annual Charity Golf Tournament to Benefit the
Connecticut Children’s Medical Center

Miss Connecticut, Lauren Betancourt, will be joining golfers for the SSIMED 3rd Annual Golf Tournament to benefit Connecticut Children’s Medical Center (CCMC). Lauren will be greeting the golfers in the morning and will also be available for photos and autographs during the day.

Last year’s golf tournament was a huge success with $15,000 raised for the Connecticut Children’s Medical Center. We are excited to be back again. Please join us on July 19 at Gillette Ridge Country Club in Bloomfield, CT to help support CCMC and the wonderful work they do with kids.

Information for Golfers
Date: Thursday July 19, 2007

Time: Registration begins at 9:30 a.m with a continental breakfast. The tournament will be a scramble format with a shotgun start at 11 am. A BBQ lunch will be served at the turn. Cocktail Reception and Dinner begin immediately following golf. Complimentary beverages will be served on the course.

Where: Gillette Ridge Country Club, 1360 Hall Blvd, Rt 218, Bloomfield, CT 06002

Gillette Ridge is situated along State Route 218 (Cottage Grove Road) primarily between the intersections of State Route 189 (Bloomfield Avenue) and State Route 185 (Simsbury Road) on the CIGNA Campus. The development is located four miles west of Exit 35 on Interstate 91.

Dress Code: Collared shirt, no denim, and soft spikes.

Cost: The cost per golfer is $150 and includes golf, cart, breakfast, lunch, cocktail reception and dinner.

Other: A silent auction will be held during dinner and raffle tickets and mulligan shots will be sold at registration.

Last year through the generosity of our sponsors and golfers, we were able to raise $15,000 to help the children at Connecticut Children’s Medical Center. This year we expect to surpass that amount with your help.

Please take advantage of this opportunity to help a worthwhile charity and gain some valuable exposure for your company. All sponsors will be recognized in the tournament booklet and will have signs showcasing your support at the tournament. All the sponsorships include the opportunity to insert an item such as a pen or golf ball in each golfers gift bag.

Titanium - $2000 Includes foursome of golfers and meals, a sign recognizing you as sponsor of a “Hole In One Contest”, a full page advertisement in the tournament booklet and recognition from the podium.

Gold - $1500 Includes twosome of golfers and meals, a sign predominately displayed near the reception and a full page advertisement in the tournament booklet.

Silver - $1000 Includes one golfer and meals, a sign predominately displayed at lunch and half page advertisement in the tournament booklet.

Bronze- $500 Includes a tee sign and a green sign and half page advertisement in the tournament booklet.

Tee Sponsor - $150 Includes a sign on the course.

New! On-the-Course Beverage Sponsor - $1500 Only 2 opportunities available. Includes signage on the course promoting you as the sponsor of one of two free beverage stands and a full page advertisement in the tournament booklet. Sponsor has the option of having a table to display your company literature along with up to two of your staff members to greet people at the table. Meals will be provided for the staff.

Raffle Donations - Every dollar that is spent by the golfers on the raffles goes directly to the charity. You can help out by donating an item to the raffle, such as gift certificate, golf bag, or any other item. Everyone one who contributes to the raffle will be recognized on signage displayed near the raffle and will get a 1/4 page ad in the booklet if the retail value of the prize is $100 or more.
